EVALUATION OF DISCHARGE READINESS IN A PATIENT WITH SECONDARY STROKE: A CASE STUDY

Abstrak

Latar Belakang: Readmisi pasca-stroke tetap menjadi perhatian penting dalam manajemen stroke, terutama bagi pasien yang mengalami gejala minimal namun menghadapi serangan berulang dalam waktu singkat. Menilai kesiapan pasien dan pengasuh untuk keluar dari rumah sakit sangat penting untuk mengurangi kemungkinan rawat inap kembali dan mengoptimalkan hasil perawatan di rumah. Tujuan: Studi kasus ini bertujuan untuk mengevaluasi kesiapan pulang pasien pasca-stroke dan keluarganya menggunakan instrumen PATH-s serta mengeksplorasi korelasinya dengan readmisi dini. Metode: Sebuah studi kasus tunggal deskriptif dilakukan pada seorang pasien pria berusia 66 tahun yang mengalami stroke iskemik sekunder dalam waktu 30 hari setelah serangan pertama. Data dikumpulkan melalui penilaian klinis dan wawancara. Kesiapan untuk pulang dievaluasi menggunakan instrumen Preparedness Assessment for the Transition Home after Stroke (PATH-s), yang terdiri dari 25 item pada skala Likert 4 poin. Penilaian dilakukan melalui wawancara terstruktur karena keterbatasan kemampuan pasien untuk menyelesaikan kuesioner secara mandiri. Hasil: Pasien menunjukkan defisit neurologis minimal selama episode stroke kedua. Skor total PATH-s adalah 69/100 dengan rata-rata 2,76/4, menunjukkan kesiapan keluar rumah sakit yang sedang. Beberapa domain, seperti pemahaman penyakit, beban pengasuh, dukungan sosial, dan kendala ekonomi, diidentifikasi sebagai yang memerlukan perbaikan. Kesimpulan: Evaluasi kesiapan pulang pada pasien dan pengasuh keluarga memiliki dampak positif dalam membantu pasien mengenali keterbatasan kemampuan mereka untuk melakukan perawatan di rumah. Hasil ini mendukung perawat dalam membuat keputusan untuk memberikan intervensi tambahan sesuai dengan kebutuhan pasien.
